At Cromehurst, teachers support families and senior students to plan for life after school. This becomes a major focus in years 10, 11 and 12. The Support Teacher Transition is involved with this individualised planning. School programs including work experience, travel training and community access are key components in preparing the students for leaving school.
Parents are encouraged to attend the anual Northern Sydney Post School Expo to learn more about possible programs and placements available for post school. Cromehurst also offers a mini expo and/or bus tour to visit providers that may be suitable for some students or providers that previous students have accessed.
The annual collaborative Individual Transition Planning includes discussing what NDIS funding the student will require as they transition to post school programs based on the goals set. For some students it is encouraged that this transition begins at the beginning of year 12 and increases through their final year to reduce anxiety and also the student to process what their new routine will look like in the following year.
Programs include:
Community Participation – this may be a day program that builds social skills and promote inclusion in their community life. Other areas the program may develop include building skills in daily living, vocation skills, recreation activities.
Australian Disability Enterprise – supported employment for people with a disability within commercial businesses.
Social Enterprise - A social enterprise is set up as a business, not a charity. A Social Enterprise puts all profits back into the business to do things that benefit society. Employees are supported to work, develop new skills and participate in their communities.
School Leaver Employment Support – two years (generally) of funding for the school leaver to have support to work towards a goal of finding and maintaining a job in the open labour market.
Disability Employment Services – organisations funded by the government to assist people with a disability to find and maintain employment in the open labour market.
